Our VBS program happens because of the work of some amazing volunteers. We have a wide variety of volunteer opportunities available. Whether you'd like to be a small group guide or lead a rotation all week, or if kitchen or check-in help is more your speed, there is room for you!
Activities for which this Medical Release is being given is described as follows: activities sponsored by Holy Cross Lutheran Church; which can be held at Holy Cross Lutheran Church, other local churches, or business establishments or organizations that would house such an activity. Youth may be asked to participate in activities that involve additional risks such as sporting activities; or service projects that require doing physically demanding work.
This consent form gives permission to seek whatever medical attention is deemed necessary by staff and volunteers of Holy Cross Lutheran Church, and releases Holy Cross Lutheran Church and its staff and volunteers of any liability against personal losses of named child.

By checking yes below, I grant Holy Cross Lutheran Church of Sioux Falls, SD the right to post pictures/videos of these activities (that may include my child) on the Holy Cross Lutheran Church website, social media, and other multimedia presentations used within the Church’s ministries.
To offset the cost of programming supplies, there is a suggested donation of $10 per child. If that doesn't work for your budget, no worries! Simply select the "None" option in the box below, or select the "Other Amount" option and give what you can. If you'd like to give more than $10 to help cover the costs for other families, great! Select the "Other Amount" option and enter the amount you'd like to give.
IF YOU WILL BE REGISTERING MORE THAN ONE CHILD: You can avoid having to charge your card more than once to submit additional registration forms. Simply choose "Other Amount" below and enter the donation amount for your total number of children ($20/2 kids; $30/3 kids; etc.) on your first registration form. Then on subsequent forms, just select "None" for donation amount to proceed without any additional payment.
